 Entry Form
Entry fee must accompany application and be payable in cash, cashier’s check, money order, traveler’s check, debit card or credit card. Personal checks will only be accepted from members of: the PGA of America; the PGA TOUR; the Nationwide Tour; or regular members of the Champions Tour. The additional $200.00 entry fee for successful Pre-Qualifiers must be paid prior to the player’s starting time in the Monday Open Qualifying. Entry fee includes: green fee; range usage on the competition day(s) and, for the successful qualifiers, the entry fee to the tournament proper.

 Contestant Information
Format:
Pre-Qualifying: 18-hole stroke play competition. All amateur and professional applicants must pre-qualify except: 2008 PGA TOUR members, 2008 Nationwide Tour members, 2008 Champions Tour regular members, and two (2) exemptions from the local PGA Section. The number of Pre-Qualifiers that will advance to the Monday Open Qualifying Competition will be 50 and ties; when more than one pre-qualifying site is used, the split for each site will be announced when the pairings are posted. When multiple sites are used, if the total number of successful qualifiers is less than 50, the number of qualifying positions will be increased equally at each site until at least 50 applicants advance. Maximum field size is 264 players for the Pre-Qualifying. If the total number of entries necessitates the use of two golf courses, requests for a specific course will NOT be granted. The Section will split the field (at random) between the two courses.

Open Qualifying:
An 18-hole stroke play event with the low 4 qualifiers advancing to the 2008 EDS BYRON NELSON OPEN CHAMPIONSHIP. In the event of a tie for the last qualifying position, there will be a hole-by-hole sudden death playoff.

Entry Deadlines:
Non-PGA TOUR Members must enter Pre-Qualifying, the entry deadline is: Friday, April 11 at 5:00 p.m. CDT.

PGA TOUR, Nationwide Tour, Champions Tour Regular Members and PGA Section Exempt professionals are exempt into Open Qualifying, the entry deadline is: Friday, April 18 at 5:00 p.m. CDT.

No phone entries will be accepted. Only entry forms received via mail, fax or online will be accepted.

Eligibility:
Professionals and Amateurs with a handicap index when applied to the qualifying course slope will generate a handicap of two (2) or less. The PGA TOUR reserves the right to verify the handicap index of amateurs and to refuse anyone not meeting the requirements.

Starting Times:
Starting times will be available after 3:00 p.m. on Saturday, April 12 for the Pre Qualifying competition. Starting times will be available after 3:00 p.m. on Saturday, April 19 for the Open Qualifying competition. Please contact the Section office; the host facilities; or check www.ntpga.com to obtain your starting time.

Withdrawals:
Applicants must withdraw prior to the entry deadline to be eligible for an entry fee refund. Contestants who withdraw during the round may be subject to disciplinary action. Refunds will be granted for withdrawals prior to the date listed on the entry form and will be charged a $25.00 cancellation fee.

Practice Rounds:
Practice rounds must be arranged with host facilities. Practice round fees will be $30.00 inclusive and includes cart. Practice rounds are available during operating hours Monday-Friday and after 2:00 p.m. on the weekends at each facility.Please note that hole #1 at Sherrill Park GC will be played as a par 4 from approximately 460 yards.


Rules of Golf:
Play shall be governed in accordance with the Rules of Golf of the United States Golf Association, as modified by the PGA TOUR Local Rules and Conditions of Competition card and supplemented by the Notice to Competitors. Questions or disputes shall be settled by the on site Rules Committee appointed by the PGA TOUR, whose decision shall be final.

Spike Policy:
The use of traditional metal spikes is permitted during the Competitions. Sherrill Park Golf Course and Firewheel Golf Park are non-metal spike facilities, all contestants MUST abide by this policy during practice rounds.

Transportation:
Players and Caddies shall walk at all times during qualifying rounds, except when permitted to ride by the Rules Committee. Players are not required to have a caddie; they may carry their clubs or use a manual or motorized pull cart. Players are permitted to ride during a Pre-Qualifying round.

Anyone seeking a golf cart due to a disability defined under the Americans with Disabilities Act (ADA) should contact the PGA TOUR Competition Department at (904) 285-3700 to obtain information on procedures for submitting such requests. All such request due to temporary injuries or impairments that do not qualify as a “disability” under the ADA will not be granted.

Appearance:
Players: While on the host facility property, players shall present a neat appearance in both clothing and personal grooming. Blue jeans, shorts, cutoffs, T-shirts or shirts with no collar shall not be worn.

Caddies:
While on the host facility property, caddies may wear either long pants or tailored solid colored knee length shorts. Blue jeans, cutoffs, T-shirts or shirts with no collar shall not be worn. Smooth rubber sole shoes are required. Golf shoes with traditional metal or soft spikes are prohibited.

Conduct:
While on host facility property during qualifying rounds; players and caddies shall conduct themselves in a professional manner. PGA TOUR reserves the right to remove a player from the qualifying competition for; non competitiveness; conduct unbecoming a professional by the player; conduct unbecoming a professional by the players’ caddie.

Reduction or Cancellation:
Any reduction or cancellation in the respective qualifying competition shall be in accordance with Reduction/Cancellation Guidelines as found in the PGA TOUR Tournament Regulations. In the event that the competition is canceled without any player having been able to play, all entry fees will be refunded. In the event the competition is canceled after play has begun, all entry fees will be refunded, minus a $25.00 administrative charge per player.
